The Dementia Data collection is a continuation of the Dementia Data 2017-18 collection. It involves collecting dementia diagnoses data broken down by age and gender, dementia diagnoses data broken down by ethnicity group, and data on dementia care plans, dementia assessments and referral to memory clinics. It also involves collecting data on patients with dementia who have been prescribed anti-psychotic drugs.
These data will be collected, along with the additional count. The additional count is entitled "The number of patients diagnosed with dementia who have a record of receiving a dementia care plan or dementia care plan review by the GP practice and who have had a medication review within the 12-month period leading up to the end of the reporting period".
